What it measures

When your body builds new bone, it releases a small protein called P1NP. Measuring it can show how actively new bone is being made.

Why it matters for women

Bone loss can speed up in the years around menopause, and osteoporosis is much more common in women. P1NP can show the building side of bone renewal. Paired with CTx, which can show breakdown, it can help a doctor see how your bones are changing and how they respond over time.

You might check it if…

  • a bone density scan showed thinning bones
  • you’ve broken a bone more easily than expected
  • a doctor is tracking how your bones respond over time

What your result may mean

High P1NP: what it may mean

High P1NP may mean your body is building new bone at a faster pace. It can happen during teen growth, while a fracture heals, with Paget's disease of bone, or as a bone-building osteoporosis medicine takes effect. Bone turnover can also speed up around menopause, so P1NP may rise along with CTx. Paget's disease can bring bone pain, while other reasons may cause no symptoms. A high value can't show on its own that bones are getting stronger, so a bone density scan can add what this test leaves out.

Low P1NP: what it may mean

Low P1NP may mean your body is making less new bone. It can be an expected effect of bisphosphonates, which can slow bone turnover, and it can also happen with age-related bone loss. Long-term steroid medicines can reduce bone building as well. Slow bone building can be silent, with nothing to feel. Change over time can say more than one number, so an earlier result or a CTx level can help.
