What it measures

Total testosterone counts all the testosterone in your blood, both free and attached to proteins. This version uses a precise lab method that stays accurate at the lower levels women normally make.

Why it matters for women

Women make much less testosterone than men, and some standard tests may not be accurate at those lower levels. This method is more precise in the female range, which helps when a doctor is looking into PCOS, a common hormone condition, or signs like acne, extra hair or irregular periods.

You might check it if…

  • you notice extra hair on your face, chest or belly
  • acne has lasted past your teen years
  • your periods are irregular or a doctor mentioned PCOS
  • the hair on your scalp is thinning

What your result may mean

High Testosterone, total: what it may mean

High total testosterone in women may point to PCOS, congenital adrenal hyperplasia (CAH) or an androgen-secreting tumor. In men, it may point to supplementation or a rare adrenal or testicular tumor. Acne, extra facial or body hair, thinning scalp hair or irregular periods can go along with it. A very high level or quickly worsening symptoms may lead a clinician to look for a tumor. Testosterone products and DHEA supplements can raise it, and levels can run higher in the morning.

Low Testosterone, total: what it may mean

Low total testosterone in women can reflect adrenal insufficiency or aging, and levels can also fall after removal of the ovaries or with estrogen birth control pills. In men, it may suggest primary or secondary hypogonadism, which can come from the testes or from the pituitary. Because women’s levels can start out low, a low value may be hard to interpret, and it may not explain symptoms such as low sex drive on its own. Testing in the morning can matter, especially for men, and a clinician may add free testosterone and SHBG.
