What it measures

This test can show your average blood sugar over the past few months. If the result lands in a certain range, the lab automatically adds a second test that looks at recent sugar spikes.

Why it matters for women

After diabetes in pregnancy, or with PCOS, blood sugar can spike after meals even when the long-term average looks fine. The add-on test may help catch those recent spikes for a fuller picture. Pregnancy can make both parts harder to read.

You might check it if…

  • you had diabetes during a past pregnancy
  • you live with PCOS or insulin resistance
  • your past blood sugar results were borderline

What your result may mean

High HbA1c with reflex to 1,5-AG: what it may mean

High A1c on this test may mean your blood sugar has run high over the past 3 months, which may point to poor long-term control. If A1c lands in a certain range, the lab can automatically add 1,5-AG, which can show sugar spikes from recent weeks that an average may hide. With PCOS or after diabetes in pregnancy, these spikes can happen even when the average looks fine. Pregnancy can make both parts harder to read, and a fasting glucose can add context.

Low HbA1c with reflex to 1,5-AG: what it may mean

Low A1c on this test may mean steady blood sugar, or it may be falsely low if red blood cells aren't living as long as they should, as can happen with hemolysis or recent bleeding. The lab may not add 1,5-AG when A1c falls outside its set range, so after-meal spikes can go unmeasured. With a history of PCOS or diabetes in pregnancy, a clinician may ask about symptoms or order other sugar tests even when this number looks reassuring.
